'Help us to help you better' mystery shopping and
customer feedback scheme

Enfield Homes is committed to providing services fairly and equally to our customers. By giving us information about yourself, you will help us to monitor who uses our services and develop new ways of working to meet your needs. The information you give is strictly confidential and we will use it for monitoring only. Completing this form, or any part of it, is voluntary. You do not have to give information if you prefer not to.

Do you think that you have an illness or disability that has a substantial and long-term adverse effect on your ability to carry out day-to-day activities?

 




The scheme is open to Tenants of Enfield Homes and members of their household. It is also open to Enfield Homes' leaseholders.

The scheme will be limited to a set number of participants and the participants will be as representative of the community as is practical to achieve. On this basis, Enfield Homes reserves the right to decline applications if the scheme is oversubscribed either in part or as a whole.

We aim to process your application within 28 days. If you are selected to participate, we will send you a pack of materials and details of optional training available to support you if you require it.

The following information is required to ensure that the mystery shopping and customer feedback scheme is representative of the Enfield Homes tenant and leaseholder community. You do not need to complete the fields marked optional if you do not wish to, and this will not affect your application. However, it does help us to monitor service delivery for this group in relation to other groups, and so is extremely helpful to us. We will mange this data in accordance with the data protection act and it will not be available to anyone who does not work for Enfield Homes.
